Head to the Middlesex Doculivery system to sign up now for electronic delivery of your W2 and new 1095-C (Employer-Provided Health Insurance Offer and Coverage Form). The 1095-C form is required in order to file your 2017 Federal Tax Return.
When you first sign in you will receive a pop up message to opt in for electronic forms. Just check Yes and your all set. You’ll need to opt in by January 1, 2018 to receive electronic delivery of your required forms. Electronic delivery ensures quick access and eliminates the potential of forms getting lost in the mail or mailed to the wrong address. Even if you have opted for electronic delivery in the past, this year you will need to re-elect that option due to new Affordable Healthcare Act forms.
Important Note: Team Members will receive an email notification from national payment when your W-2 is ready to print or view on Middlesex pay web site. For security reasons, W-2’s will no longer be available to be emailed.
On the Doculivery site you can:
- Verify your mailing address. Contact Human Resources to update your mailing and email address (before December 31st)
- Print your Tax Information for 2018, in mid-January
- OPT in 1095-C & W2, no later 1/1/2018

Click here to print a Change of Address Form return this form to Human Resources for processing.
